However, I noticed that during a phone call on iPhone, if an alarm goes off, the sound will be low compared to when you’re not making a phone call.
At first, I thought that the volume was low, but when I checked the phone, I saw that it was indeed high.

Definitely the other person will hear the alarm, and which will be very irritating to the person’s ear.
On facetime call, you can use the standard mic mode or the voice isolation mic mode during your facetime call.
For me, I always recommend using the voice isolation unless you want to hear the background or what is going on in the other person’s background.
